This utility is getting hard to find, thank you to Vetusware for hosting it! It comes with some very detailed documentation, and is the right tool if you have serious drive problems. I had to strip out the documentation to get this to fit on a bootable 1.44M floppy disk, but it did fit, with 159K free. There seems to be a Windows program in there that I could remove for more space. The DOS version works as intended (tested on a 486 with 6MB RAM.)
