Here is a general approach that can work if you are lucky. Make a directory under dos/Windows and place a ccpm.sys file there. Then look for a format.com or format.exe file from one of the distributions and put that there also. Use the format command with /s and I think /b to create a bootable diskette. I have had some luck with this technique.
